Received an RFP?

Important Applicant information:

  • Read our REQUIREMENTS page before applying, as they have changed.
  • All Requests for Proposal will be emailed by June 30th.
  • SUBMIT Grant Application for invited proposals BEFORE August 1st of this year. Applications should be concise, specific, and limited to the allotted space on the application.
  • Grant awards will be announced after December 31st, and will be distributed during the first quarter of the following calendar year.

All of the following documents must be included:

  • Completed application.
  • Copy of 501(c) (3) determination letter.
  • Audited financial statements for the most recently completed fiscal year. If an audit was not done, attach unaudited financial statements.
  • Form 990 for the last fiscal year. If there is no Form 990, attach Form AG990-IL, Illinois Charitable Organization Annual Report.
  • Budget for current fiscal year and a list of the sources of revenue for the organization.
  • Detailed budget for this grant project including other sources of revenue.
